Every payer we cover, and every source behind it.
This is the list, nothing held back for a sales call. Each payer’s policy sources refresh on their own cadence and are flagged — not silently served — once they go stale. If a criterion in one of your appeals came from a source, you can open that source.

Each source carries its own refresh cadence. When a payer republishes a policy, the source is re-indexed and the criterion’s refresh date moves. If a refresh fails, the source is marked degraded and the criteria drawn from it are flagged in the case — never served silently as current.
If your payer mix includes a plan that isn’t on this list, say so in the pilot and we’ll tell you honestly whether onboarding it is a week or a quarter. New payer sources are added without a code deploy, but we won’t claim coverage we don’t have.
